player listed. I accept that I am responsible for the accuracy of all registration details and compliance with League Constitution Clause X and that
the omission, incorrect or false completion of the details may render the registration ineligible within the meaning of SYL constitution. By signing
this form both player & Club agree to abide by all registration rules as listed in the appropriate section of the current handbook / website and that
ignorance or lack of understanding of these rules is not a valid reason for the SYL not applying them or indeed for any appeal against
implementation of the sanction the SYL may apply for breaking those rules. This expressly covers any aspect of Professional Academy Player
Status and International Clearance. As Club Official I accept responsibility on behalf of the Club for gathering & recording any known serious
medical conditions of the Player and emergency contact details of the Player’s parents or guardians. These details must be available at matches
and training events the Player attends within the management of the Club or Competition.

GUIDELINES FOR COMPLETING THIS FORM
1)
Enter your Club name, Team name (“A”/ “B”/Reds/Blues etc) and Age Group on this form.
2)
Glue (not staple) trimmed photograph of players face in box. If folding this form be careful not to crease photos.
3)
Player details must be entered and player must sign to confirm desire to play for your team. You cannot sign on their behalf.
4)
Proof of date of birth is required for player being registered. This can be last seasons SYL ID card or photocopy (not originals) of
birth certificate, passport, medical card, driving licence.
5)
Players will not be registered until their details have been added to the Leagues Prawn Sandwich administration
website. Please enter the PS Registration Number in the above box.
6)
Club/Team secretary must countersign the form to confirm the details are correct and that Club agrees to all Registration Rules.
7)
Once completed, post by normal 1st class mail (not registered, recorded, or any method that requires a signature) to:

ENCLOSING:(A) Correctly completed Registration Form;
(B) Stamped self-addressed envelope for the return of the ID Card – with sufficient postage for the size of envelope being used;
(C) Proof of Players Date of Birth
(D) Registration Fee as follows: (cheques / postal orders payable to “Surrey Youth League”)
U7/U8 – Free until 30th November thereafter £2.50
All Other Age Groups £7.50 until 11th September thereafter £5 per player
NB: Players can only play when in possession of a valid Registration Card. Postal Applications only please – no personal callers.
