Context: Enabling Active Communities
(EAC) Policy
• GCC Active Communities policy adopted in April 2015
• Enabling Active Communities (EAC) joint policy
adopted by the Health and Wellbeing Board in July
2015. Agreed principle as the foundation for
collaborative working:
“To build stronger, more sustainable
communities and in turn improve the health and
wellbeing of local people, we will draw upon,
and stimulate the provision of, the diverse
range of assets within each local community.”

‘Engaging with Communities’ Principles
• An asset based approach: recognising and building on strengths within local
communities
• A needs-based approach : giving priority to those whose need is greatest
• A shared approach: developing shared solutions that take account of what
works in each community
• A local approach: local people decide the outcomes which matter the most
for their community
• An empowering approach: that empowers the individual to take control of
their health and wellbeing and promote self-care
• A partnership approach: working with all partners to harness existing
capacity in the community and grow the community workforce in a sustainable
way
• An inclusive approach: ensuring that under-represented groups are
considered and that communities include all views and backgrounds

Social Prescribing
The journey so far:
Social Prescribing (SP)
‘Social prescribing is a mechanism for providing one to one
support, linking patients with non-medical needs to sources
of support within a community’.
• Launched by Gloucestershire Clinical Commissioning Group in 2014 in 2
pilot areas
• Aimed at patient groups who are frequent attenders in primary care;
vulnerable and at risk groups; people with long term conditions
• From March 2016 patients in all 81 practices in Gloucestershire have access
to social prescribing
• Accepts referrals from GP practice staff , staff in 21 Integrated community
Teams and staff in community hospitals
• 24 staff across the county. Based in GP Practices.
The story so far:
Village and Community Agents
‘A service to provide older, frail and vulnerable people easy
access to a wide range of information and local community
connections to meet the needs to remain living
independently.’
• Established in 2006 delivering in rural areas only
• Extended to become countywide in 2012
• Open access for over 50s but majority of clients are over 70
• 38 community agents, average 10 hours per week, based in the
community.

Where we want to go next?
The Community Connectors (Social Prescribing and
Community Agents) service aims to increase the
wellbeing and resilience of the local community and
reduce the reliance on statutory services for all ages
through:
• Improving the quality of life of individuals and increasing their
knowledge, skills and confidence to enable them to take control of
their wellbeing, live independently and improve their health outcomes
• Utilising and increasing the strengths and capacity of the local
community through closer working with the voluntary, community and
social enterprise (VCSE) sector
• Promoting innovation and development of local solutions by local
communities

• Agile response to emerging and fluctuating patterns of
need
• Service stability, resilience and contingency
• To minimise infrastructure and on costs – to enable
maximum investment in front line

How we plan to achieve it:
• Proportionate and effective performance and contract
management
• Indicative contract value in region of £650k - £800k per
annum
• Contract length: either 3 years plus option to extend for
further 2 years, or 5 years plus 2 years
• How we pay: block payment, rate per intervention or
other?
