The first field below is for your title. It should be entered using all CAPITAL letters (the rest of the page is in mixed
case). For example,

The second field is for your name. For example,
Rodney Worthington Smythe, Jr.
The next two fields are for your undergrad (or first) degree, institution, and year of graduation. Do not abbreviate. For
example,

The next two fields are for your masters (or second) degree, if you have one. Do not abbreviate. For example,
Master of Science

Following the line "For the Degree of Doctor of Philosophy in", you should fill in the full and official name of your
major, not your department (e.g. Electrical Engineering, NOT Department of Electrical Engineering). Do not abbreviate.
For example,
Electrical Engineering or Linguistics or Clinical-Community Psychology
The next field contains the full and official name of your school or college. Do not abbreviate. For example,
School of Public Health or College of Arts and Sciences
The next field contains the year of your graduation.
In the signature area there are two blank fields. DO NOT FILL IN THESE FIELDS WITH NAMES. If your Major
Professor IS NOT the Chair of your examining committee, leave the field under "Major Professor" blank, and insert
"Chairman, Examining Committee" under the top right hand line. If your Major Professor IS the Chair of your examining
committee, insert "Chairman, Examining Committee" under "Major Professor", and insert "Committee Member" under
the top right hand line.
Do NOT print this form on a color printer.
