Amendment #4 to P-270 “Voice System Upgrade Replacement”
This addendum becomes part of the Contract Documents.

1)
Extend RFP due date to Monday, June 16, 2008, 3:00 pm local time.
2) Q: There are questions in the RFP that require a great deal of information to answer and they take up
a lot of pages with details that will not directly influence the decision process. There are three specific
questions in my response that are causing me this problem. The first is the complete parts list, when
adhering the to 12pt font rule, the parts list is a dozen pages, then the complete environmental specs for
these parts, is another 5 to 7 pages, and lastly the detail project plan and milestones that are requested
are another dozen pages. So I am at 50% of the page limit and have answered only 3 of the 62
questions. So my question would be if these three items, which are primarily just list’s of information be
included as appendix’s referenced in the body of the RFP but not be included towards the total page
count?
A: CNM will accept the following changes to the 30 page limitation the first section
“ Proposed Solution Criteria” as follows:
o Complete parts list (All required hardware & software components) may be submitted as a
separate Appendix to this section. This Appendix will be limited to 10 pages and will
not count against 30 page limitation for the proposed solution criteria
o Detailed environmental requirements – cooling, foot-print and power include type and number
of receptacles (for example: L-5 30amp).
o may be submitted as a separate Appendix to this section. This Appendix will be limited
to 10 pages and will not count against 30 page limitation for the proposed solution
criteria
Note: The detailed project plan should be included within the 30 pages maximum proposal solution
criteria section.

If possible can we 1) get an LOA (Letter of Agency) to get the reports from AVAYA directly or 2)
Get the following reports from your staff: List Config all, Display Capacity, and Display system
parameters/ customer options.
This information will give us the ability to provide an accurate migration path for your existing
AVAYA equipment inclusive of all hardware and software. For example in the walkthrough you

mentioned some restrictions on licensing, it is important to be able to calculate how much of each type of
licensing you currently have and where it is assigned to. (Standard edition RTU’s vs. Enterprise Edition
RTUs).

Display Capacity, Display System Parameters / Customer Options: Screen shots of the requested

4) Q: The following requirement in the supplemental conditions for financial statements is on page five,
item 15. Can this be removed if possible?
FINANCIAL STATEMENTS REQUIRED. All vendors are
required to submit with their proposal their most recent audited
Financial Statements.
A:
Change the requirement to submit most recent financial statement to the following:
FINANCIAL STATEMENTS REQUIRED. If requested in writing by CNM , All vendors may be required to
submit their most recent audited Financial Statements. Vendors are not required to submit their Financial
Statements with their written proposal submittal.
5) Attached is the sign in list for the second mandatory meeting on June 2, 2008. Note: Vendors are
required to attend at least one mandatory meeting.
