This addendum becomes part of the Contract Documents.
Offerors are required to acknowledge receipt of this amendment/addendum in the
space provided on page 16; Section E. SIGNATURE OF FIRM'S AUTHORIZED
REPRESENTATIVE FOR RFP #P-349.
Questions / Clarifications as follows:
QUESTION 1:
Name of the current vendor and start date of their current contract.
ANSWER:
Outcomes Inc. has been providing the EAP services for CNM since April 15,
2008.

QUESTION 2:
Confirmation that the program design and its elements in the Request for
Proposal are the same as what is now contracted.
ANSWER:
The requested program design closely follows the current contracted services.
However, there are some deviations which we have noted here. The current
contract includes 4 hours per month, each month per contract year, formal
conflict resolution/formal mediation services as requested by CNM human
resources department which has not been specifically requested in this RFP. The
current contract does not include the following from the 14 items listed in the
scope of services – #12, #13

Proposers may include information in their proposal of additional services

information so long as the proposal does not exceed the maximum pages

Can CNM provide an employee census (zip codes only) in order to produce a
Geographic Access report?
ANSWER:
The majority of CNM employees are located in the greater Albuquerque / Rio
Rancho area. If proposers wish to provide a network access report, it should be
based on that metropolitan area.

QUESTION 15:
How many hours of on-site Critical Incident Stress Management sessions, if any,
were conducted over each of the last three contract years?
ANSWER:
CNM is providing additional utilization data as an amendment to the RFP –
including Critical Incident Stress Management (CISD) session statistics. The data
will be posted to the CNM purchasing website under Request For Proposal P349.

What is the total number of training hours included in CNM’s current contract on
an annual basis?
ANSWER:
CNM’s current EAP contract includes up to 40 hours per contract year for
employee training. The contract also includes up to 40 hours per contract year of
group intervention/critical incident stress debriefing (CISD) available on an oncall basis, 24 hours per day 7 days per week for crisis intervention.

ANSWER:
Yes, answers to questions will be released via RFP amendment as close to the
tentative schedule provided.

website as an amendment to the RFP.

QUESTION 20:
Clarification on – in regards to the (5) additional sessions that members who
experience a Critical Incident receive: 1) please confirm that these additional
sessions are also paid for by CNM and 2) is there any specific utilization data as
to how many of these sessions have been used by members historically?
ANSWER:
These additional sessions are provided at no cost to the employee. The cost for
these sessions should be included in the PEPM fee for the program. CNM is
providing additional utilization data as an amendment to the RFP; however, there
is not data available on the specific utilization of the 5 additional sessions in the
case of a critical incident. The data that is available will be posted to the CNM
purchasing website under Request For Proposal P-349.

proposal so long as the entire proposal does not exceed the page limit specified in
the RFP.

In regards to the stated utilization of 3.3%, could you please provide more clarity
around the specifics of this? For example, is this newly opened EAP cases only
or does it include other items such as web hits, etc.?
ANSWER:
The RFP provides high level utilization data for Utilization Rate – Active Cases
(active cases/employees) and Utilization Rate – Covered Lives (persons
services/covered lives). CNM is providing additional utilization data as an
amendment to the RFP. The data will be posted to the CNM purchasing website
under Request For Proposal P-349.

CISD session hours provided. CNM is providing additional utilization data as an

posted to the CNM purchasing website under Request For Proposal P-349.

QUESTION 39:
In regards to on site seminars (i.e. attendance at the health fair), would CNM like
the cost for that built into the PEPM or would it be on a fee for service basis?
ANSWER:
This would be a built in cost to the PEPM.

For the (5) additional sessions for those who have been affected by a CIR, would
CNM be paying for these on a fee for service basis? If not, what is the
arrangement to be?
ANSWER:
No, CNM will not pay for these sessions on a fee for service basis. These
additional sessions are provided at no cost to the employee. The cost for these
sessions should be included in the PEPM fee for the program.

What does the current provider charge for each additional session beyond the
original 6?
ANSWER:
The current EAP plan provides 6 sessions per issue at no cost to the employee or
family member. Should additional sessions be needed, the cost for those services
is the member’s responsibility and may be covered under the member’s health
insurance. CNM has not contracted or negotiated the rates for additional sessions
outside those covered by the EAP plan.

QUESTION 47:
On page 3 of 21, paragraph 1.a., it states "CNM desires that your response be in a
point by point format for the Services Types being offered." Could you please
clarify this? Does it mean that we should respond to each numbered item in the
sections 2.3.1 through 2.3.6? Or does it mean something else?

ANSWER:
Yes, proposals should include complete and organized responses to the items
listed in sections 2.3.1 through 2.3.6 (i.e. the questions provided under
“Evaluation Criteria”)

QUESTION 48:
On page 12 of 21, item #5 refers to providing "six (6) sessions". Are there any
time limits or other types of limits regarding these six sessions? For example,
"six sessions per year", or "six sessions per issue"?
ANSWER:
The six sessions are provided per issue of the employee or family member.

QUESTION 49:
Regarding 2.3.4, Section IV. Cost for services: If we would like to submit
alternative cost quotes that vary in their Scope of Service details, can we include
those alternatives, clearly labeled, in our response to this section? Or would
offering an alternative cost quote require an entire separate alternative proposal?
