MPA ADVISEMENT FORM
Before registering for classes, students must complete the following form in consultation with the
Director of the MPA program, Dr. Chris Witko ([email protected]). After completing this form
students must submit the completed form to the Graduate Coordinator, Ms. Debby Tiemeyer
([email protected]) in order to register for classes.
Once submitted, any deviations from the proposed courses will require the approval of the
MPA Director.
