Due to limited space, we are requesting that ONLY the parent/guardian and student be present to register.
Please expect to be at the school for up to three hours on the day you attend. To finalize enrollment, please bring
the following documents with you:
1.
Parent must bring a photo ID such as a state ID or driver’s license. Guardians must bring
guardianship papers issued by the Circuit Court of Cook County in addition to the photo ID.
2. Proof of address required (ex: current utility bill, MediPlan/Medicaid card, voter registration card,
court documents, IL Dept. of Public Aid card, stamped USPS change of address form, or check from
the State of IL/Social Security.
3. If you child has an IEP or 504 Plan, please bring a copy.
