Changing a US Address
In order to be in compliance with immigration regulations, all international students and scholars holding and F-1
or J-1 visa MUST do the following:
Inform the Office of International Programs of any address change within the United States within 10 days of
moving. This can ONLY be done by filling out, signing, and submitting the OIP Change of Address Form to the
Office of International Programs. OIP Change of Address forms are located in the lobby of the Office of
International Programs (Polsky Building 483) and can also be downloaded at
http://www.uakron.edu/oip/immigration/forms.dot. This requirement includes students on Optional
Practical Training.
Note: In order to keep your records updated, it is also your responsibility to change your address with the
Registrar’s Office. This can be done by completing a “Change of Address Form” in the Registrar’s Office (Simmons
Hall) or on Zipline at http://zipline.uakron.edu. Please note that the Registrar’s Office “Change of Address Form” is
different than the Office of International Programs’ “Change of Address Form”.
Summary of what to do when you move to a new US address:
1.
Fill out an OIP “Change of Address Form” and return it to the Office of International Programs within 10
days of the move.
2.
Change your address with the Registrar’s Office by filling out the “Change of Address Form” in Simmons
hall or by updating your personal profile in Zipline at https://zipline.uakron.edu.
***Note: In addition to the above requirements, all international students and scholars who do not hold an F-1 or
J-1 visa must also submit changes of address to the United States Citizenship and Immigration Services (USCIS)
through the completion of the AR-11 form.
Travel Signature
If you plan to travel outside of the USA, you need to check page 3 of your I-20 (front page of DS-2019) to be sure
that it has been signed and dated within 1 year of the day you plan to reenter the US**. If the signature will be
expired before you plan to reenter the USA, you need to request a new travel signature, by completing the
“Change of Address/Travel Signature Request Form” and submitting it to the Office of International Programs
along with your most recent, original I-20.
**If you will be applying at a US Embassy to renew your visa, you will need a new travel signature even if the
current signature is still valid.
Requirements for Reporting Upon Reentry:
If you have reentered the USA, you MUST bring a copy of your new I-94 Arrival-Departure Record (as well as
copies of your visa and passport if you renewed them) to the OIP as soon as possible.

***Note: Please check the expiration date on your visa stamp in your passport if you are planning to travel outside
