I request review of my application for full admission to the School of Social Work at the end of
the semester indicated below.

I read and understand all requirements for full admission in the School of Social Work
described in the Pending Social Work Major Checklist.
My application for full major status will be reviewed after completion of all full major
admission requirements.
Akron campus admits students as full majors in all semesters. Wayne campus admits
students as full majors for spring semester only.
To receive permission for conditional registration in my first semester of full major
classes, I must meet with and receive permission from the Pending Student Advisor or
another appropriate academic advisor designated by the school.
I understand that I am responsible for notifying the Pending Student Advisor when my
application is complete. If I need to change the requested date for any reason, including
missing the application deadline, I must submit a new Request for Application Review
form to the Pending Student Advisor.
