Unusual Enrollment History Appeal 2016‐2017 Student Name: _____________________________________________ SJC ID Number: _________________ Part 1. Instructions and Definitions: Students that have been denied eligibility based on the failure to adequately explain unusual enrollment history (semesters of receiving Pell Grant but not earning any academic credit) are eligible to appeal the decision to the Director of Financial Aid. Extenuating or mitigating circumstances are unforeseen situations that are beyond a student’s control that could create an undue hardship and hinder a student’s ability to earn credits in an academic term. Examples include, but are not limited to death of an immediate relative, or severe personal injury or illness. The following reasons may NOT qualify as extenuating circumstances. Examples include, but are not limited to: course difficulty; single parent; having to work or pre-existing work schedule; teaching style or disagreements with an instructor; changing majors, dropping courses to avoid a bad grade; brief medical situations, etc. The appeal review will take into consideration: The original review of eligibility, the Unusual Enrollment History Review form and all supporting documents; all official academic transcripts and academic history; a student’s appeal application and statement (explanation) for all semesters in which financial aid was received but academic credit was not earned; any supporting documentation; etc. The appeal decision is final. Students awaiting an appeal decision not eligible for financial aid and are responsible for making payment arrangements for their classes. Part 2. Explanations and Documents for Appeal: For an appeal to be considered, students must include ALL items listed below. 1. List all schools attended and dates of attendance in academic years 2013-2014, 2014-2015 & 2015-2016 __________________________________________________________________________________________________________________ __________________________________________________________________________________________________________________ ____________________________________________________________________________________ 2. Attach an unofficial copy of academic transcript(s) from all schools attended in years 2013-2014, 2014-2015 & 2015-2016 Note: Official transcript may be requested if not already on file with SJC Enrollment Services Office 3. Attach complete explanations for ALL semesters on your transcript(s) in which you DID NOT earn any academic credits. See definitions above for potential acceptable and non-acceptable reasons. New or expanded explanations that was not included with the original Unusual Enrollment History Review Form. 4. Supporting documentation. Each appeal must include dated documentation to support the reason of appeal and /or how these circumstances kept you from earning any academic credit in a semester. See definitions above for potential acceptable and non-acceptable reasons. New or expanded documents that was not included with the original Unusual Enrollment History Review Form. Any letters should be on letterhead paper, signed, dated and come from authority figures. Letters from family members or personal friends are not acceptable. Lack of documentation could result in denial of your appeal. Part 3. Appeal Decisions: Approved: Student will be immediately eligible for financial aid. Denied: Student is ineligible for financial aid. Students may apply for consideration of reinstatement of financial aid eligibility following completion of reinstatement criteria found on the financial aid website at: www.sanjuancollege.edu/financialaid. Part 4.
