Independent Students:
List the people in your household in the chart below. Include the following:
a)
Yourself,
b)
Your spouse, if married. (Boyfriends and/or girlfriends should not be included unless YOU provide
financial support as explained below in letter ‘d.’)
c)
Your children, if you will provide more than half of their support from July 1, 2016 through June 30,
2017; and
d) Other persons if they now live with you, and you provide more than half of their support, and you will
continue to provide more than half of their support from July 1, 2016 through June 30, 2017.
Dependent Students:
List the people in your parents’ household in the chart below. Include:
a) Yourself, even if you don’t live with your parents,
b) Your parents,
a. If your parents are married, list both.
b. If you live with one parent and he or she has remarried, include the stepparent information.
c. If your biological parents are unmarried but living together, then list both parents information.
d. If your parents are divorced/separated, but living together, list them both.
e. Boyfriends or girlfriends of a single parent should only be listed if your parent is the one providing
financial support for them (as in letter ‘d”). Otherwise boyfriends or girlfriends should not be
included. Only your biological mom or dad.
c) Your parents’ other children if:
a. Your parents will provide more than half of their support from July 1, 2016 through June 30, 2017, or
b. The child would be required to provide parental information when applying for Federal Student Aid
(under the age of 24) and
d) Other persons if they now live with your parents, your parents provide more than half of their support and will
continue to provide more than half of their support from July 1, 2016 through June 30, 2017.
Write the names and ages of all household members. Also identify the college for any household member, excluding
your parent(s), who will be attending college at least half-time between July 1, 2016 and June 30, 2017, and will be
enrolled in a program that leads to a college degree or certificate. If you need more space, attach a separate page.
