DUPLICATE DIPLOMA/CERTIFICATE REQUEST The University of Akron Office of the University Registrar As a graduate of The University of Akron, you may request, free of charge, a single duplicate diploma/certificate by properly completing this form and submitting it to the Office of the University Registrar. Please be advised that the duplicate diploma/certificate will bear the signatures of current university officials as well as a notation indicating that the document is a duplicate. Your duplicate diploma/certificate will be mailed via the United States Postal Service within ten (10) business days of receipt of your request.
