This is your academic improvement and success plan. The Student Achievement Center Staff is
committed to your academic success and available to assist you throughout your academic
career. You are expected to complete the following objectives in order to help you continue on
a successful track towards obtaining your degree/certificate/academic goal.
Student Achievement Center Policies
1. You are expected to complete all tasks on your Achievement Plan during the current
semester.
2. You are allowed three (3) no shows with Student Achievement Staff before you will be
automatically removed from receiving our services. You will be able to begin receiving
our services the following semester.
3. You are expected to attend all scheduled appointments with Student Achievement
Center Staff. IF you cannot make it in please notify us as soon as possible by calling or
emailing.
4. You are expected to be on time for all scheduled appointments with Student
Achievement Center Staff. Ten (10) minutes past your scheduled appointment time will
be considered a no show and you will be asked to reschedule.
5. Remember we are here to help you; however, you are primarily responsible for your
academic success.

1. Attend all classes regularly. Contact your instructor(s) when you are unable to attend
(ahead of time if possible).
2. Attend all scheduled follow-up appointments with your Achievement Coach. Contact
Kathy Jensen (566-3378) if you are unable to make an appointment. Reschedule if
possible at that time.
3. Complete all assignments and tests in every class on time.
