2011 Human Services Report FUNDING SNAPSHOT In the 2010 fiscal year (October 1, 2010 – September 30, 2011), our funding recipients served a total of 10,232 low-moderate income individuals in Fort Collins. Affordable Housing and Human Services gave: - $567,959 in the form of 20 grants to 18 local non-profit agencies, - $178,358 in Federal Community Development Block Grant (CDBG) dollars, and - $389,601 in local Human Services Program (HSP) money. For the 2011 fiscal year (October 1, 2011 – September 30, 2012), City Council approved funding by giving: - $689,458 in the form of 24 grants with 22 local non-profit agencies (an increase of $121,499 dollars), - $149,124 in Federal Community Development Block Grant (CDBG) dollars, - $389,601 in local Human Services Program (HSP) dollars, and - $150,733 in Keep Fort Collins Great dollars. FUNDED AGENCIES – FISCAL YEAR 2011 Alliance for Suicide Prevention of Larimer County - allianceforsuicideprevention.org Our mission is to prevent suicide in Larimer County by raising awareness, educating and training youth and adults about depression and suicide, and providing resources and support to those who have been impacted. FUNDING: $3,000 R.A.P.P. AND HOPE FOR TODAY PROGRAMS B.A.S.E. Camp of Fort Collins – mybasecampkids.org Provides extended learning and child care in 22 elementary schools in Fort Collins through purposeful planning, homework assistance, nutritious snacks, extended learning, and safe, fun activities. We serve approximately 2,700 elementary students per year. Funding: $57,000 for Sliding Scale Fee Tuition Assistance Boys & Girls Clubs of Larimer County – bgclarimer.org Our mission is to provide an environment - people, place, and programs - that helps kids develop to their full potential. We believe that every child has the potential to BE GREAT, and we strive to build incredible kids who will become caring, responsible citizens. FUNDING: $18,644 FOR AFTER-SCHOOL AND SCHOOL BREAK CHILD CARE & YOUTH PROGRAM CASA, Inc. of Larimer County – fortnet.org/casa We provide a voice in court for abused and neglected children and a safe place in the community for conflict-free family interaction. FUNDING: $9,360 PROGRAM SUPPORT HUMAN SERVICES REPORT 2011 1 Catholic Charities and Community Services (CC) – ccdenver.org CC’s primary purpose is to work with vulnerable individuals and families to meet basic needs, alleviate poverty and promote self-reliance by providing supportive, safety-net services. CC serves people of all faiths, backgrounds and cultures. FUNDING: $11,331 SENIOR OUTREACH PROGRAM AND $40,000 FOR THE MISSION HOMELESS SHELTER AND SUPPORTIVE SERVICES Crossroads Safehouse – crossroadssafehouse.org Crossroads is the only comprehensive domestic violence shelter that serves all of Larimer County. Our service provision package includes emergency safehousing; 24/7 on-the-scene response to domestic violence crimes; individual and group counseling for children, teens, and adults; music therapy for children; prevention education; educational enrichment; legal advocacy and representation; transitional housing; mental health therapy and pet foster care. FUNDING: $42,202 ADVOCACY PROGRAM FOR BATTERED PERSONS Disabled Resource Services (DRS) – fortnet.org/drs We help clients deal with the challenges of disability and move towards greater empowerment and independence where they have more control over their lives and the circumstances influencing them. FUNDING: $28,442 ACCESS TO INDEPENDENCE PROGRAM Education and Life Training Center (ELTC) – eltcenter.org ELTC provides affordable education and employment skills training to adults. FUNDING: $19,483 EMPLOYMENT SKILLS TRAINING Elderhaus – elderhaus.org Elderhaus provides a therapeutic daytime program for adults. These low-income clients are disabled with special needs that must be supervised for their safety. In addition, the program helps them to increase their independence and enables them to face and overcome their physical, mental, emotional and social challenges. FUNDING: $23,592 MINDSET THERAPY CENTER ACTIVITY PROGRAM The Family Center/La Familia (TFC) – thefamilycenterfc.org TFC is a high-quality, bi-lingual education center that seeks to strengthen and stabilize low-income, at-risk families with children under the age of 18. We provide on-site supportive services, coordination and collaboration with other service providers. Through the referral process, we also connect families to basic needs, training, and education. FUNDING: $30,000 SLIDING SCALE FEE TUITION ASSISTANCE Food Bank for Larimer County – foodbanklarimer.org The Food Bank reduces hunger in our community by distributing food to single-parent families, the working poor, senior citizens, youth in crisis, the disabled and the homeless. FUNDING: $21,000 KIDS CAFÉ Homelessness Prevention Initiative (HPI) – homelessnessprevention.net HPI provides rental assistance and resource referrals to low-income households experiencing a temporary financial emergency that puts them at imminent risk of homelessness through eviction. FUNDING: $45,000 RENT ASSISTANCE 2 HUMAN SERVICES REPORT 2011 Larimer Center for Mental Health (LCMH) – larimercenter.org | healthdistrict.org | fchousing.com The Community Dual Disorders Treatment (CDDT) Team is a multi-disciplinary team comprised of members from Larimer Center for Mental Health (LCMH), the Health District of Northern Larimer County, and the Fort Collins Housing Authority. LCMH’s purpose is to provide unsurpassed behavioral health prevention, intervention and treatment services to vulnerable residents of Larimer County. The Health District’s mission is to improve our community’s health status. The FCHA’s purpose is to provide and promote safe and affordable housing, economic opportunity and a living environment free from discrimination. Funding: $14,000 Community Dual Disorders Treatment Program The Matthews House (Live the Victory) – TheMatthewsHouse.org The mission of the Matthews House is to empower young adults who lack significant family support to transition successfully to adulthood. We provide the resources and support necessary for them to take control of their lives, shape positive futures for themselves and become successful contributors to their communities. FUNDING: $27,639 YOUTH TRANSITION PROGRAM Neighbor to Neighbor (N2N) – n2n.org The mission of Neighbor to Neighbor is to open doors and advance lives by providing housing counseling, supportive services, and multi-family affordable housing. FUNDING: $40,175 HOUSING COUNSELING AND $25,000 RENT ASSISTANCE Northern Colorado AIDS Project (NCAP) – ncaids.org The mission of NCAP is to improve the quality of life for people living with HIV/AIDS, and to help reduce the spread and stigma of the disease. FUNDING: $24,500 CLIENT SERVICES AND HOMELESSNESS PREVENTION Project Self-Sufficiency (PS-S) – ps-s.org PS-S assists low-income, single parents in becoming financially independent through education, career development, and the removal of barriers to self-sufficiency. Services include case management, career planning, problem solving, and an array of resources for families. FUNDING: $22,000 SERVICES FOR SINGLE PARENT FAMILIES Rehabilitation and Visiting Nurse Association (RVNA) - rvna.info RVNA provides acute and long term home care services in the client’s place of residence. Services include: skilled nursing, physical therapy, occupational therapy, speech therapy, medical social services and personal care. FUNDING: $35,000 HOME HEALTH CARE SCHOLARSHIPS Respite Care Respite Care’s purpose is to provide short-term care for children with developmental disabilities as well as other services and supports that provide respite for families, enabling them to enhance their quality of life. This support is available to families 24/7 year round, and care is provided for children from infancy to age 21 who have physical, emotional and/or sensory disabilities. This is done in a culturally and economically diverse environment ensuring a climate for individual growth, comfort, and security. FUNDING: $60,000 SLIDING SCALE FEE TUITION ASSISTANCE Volunteers of America Colorado Branch (VOA) – voacolorado.org VOA seeks to identify and serve the basic needs of the most vulnerable individuals and families in the community. In Larimer County we focus our services on low income seniors with the goal of keeping them in their homes as long as possible. FUNDING: $29,200 HOME DELIVERED MEAL SERVICE Women’s Resource Center - womens-resource.org The Women’s Resource Center is dedicated to improving breast, reproductive, and dental health for medically underserved women. We provide community-specific outreach, women’s wellness education, connection to free or reduced-cost medical services, and patient navigation to reduce barriers to care.
