Instructions - How to book your flu vaccination appointment
Step 1
Click on the link below to access the CSU Flu Vaccination webpage.

Step 2
Once you have accessed the Flu vaccination webpage, select your chosen campus, then follow step 3
to step 5.
Step 3
Click on the drop down box under QUANTITY next to your preferred time to receive the flu
vaccination, and then click on REGISTER.
Step 4
Within 15 minutes, fill out the required information (First Name, Surname & Email Address) and then
click COMPLETE REGISTRATION.
Step 5
The next screen will show confirmation of your booking details. You will also receive an automated
email from Australian Vaccine with all the details on it. On the day of your appointment, you will
need to complete a vaccination consent form and present it to the nurse prior to receiving the
injection. These forms can be accessed from the link highlighted below.
If for some reason you need to cancel your booking, please see the steps below.
NOTE:
Immunisations must be paid for at the cashier’s office PRIOR to your appointment and a receipt
presented at the time of vaccination; if you arrive for your immunisation without a receipt you will
not receive a vaccination.
