Application to Graduate
1. For the December ceremony, please return this application or apply online at: http://i.waikato.ac.nz as soon as possible and
before 14 October 2016. For those who wish to graduate in absentia, please return this application as soon as possible to be
included in the next available ceremony/Council meeting. Closing dates for each ceremony/Council meeting are on the back of
this application form.
2. Please post to: The Graduation Coordinator, The University of Waikato, Private Bag 3105, Hamilton or hand it in at the Student
Centre, Level 2, Gate 5, Hillcrest Road, Hamilton or email [email protected].
3. All applications will be acknowledged.
4. Please note that any outstanding cross-crediting must be completed before your Application to Graduate can be finalised. If
you received notification of this during the year and have not attended to it please do so immediately.
5. Please note that any outstanding fees/fines must be settled before your Application to Graduate can be finalised (no later than
14 October 2016).
6. If the Graduation Office does NOT receive your Application to Graduate form by 14 October 2016, your conferment will
automatically be deferred for up to a year

changed your name and wish to have your qualification conferred or awarded under your new name you must attach verified
documentary evidence of your name change, e.g. marriage certificate or deed poll certificate, passport, birth certificate

to the ceremony
