Reporting a Hazard – Quick Reference
To report a Hazard via the CSU Public Portal go to
https://csupublicportal.scrimonline.com.au/Main.aspx#
From the menu bar select Report a Hazard (see Figure 01 below)

Complete all fields as required.
Helpful Hints:
Mandatory - All fields marked with a red asterisk ( * ) must be completed before the report can be submitted.

Location Details – This is the location of the hazard. If the hazard is not related to a Faculty or Division, then
choose the Faculty /Division you belong to. ‘Further Location Details’ should be used to help describe the site
and location details in more detail.
Hazard Details – Provide as much details as required. If a BIEMS request has been submitted, please note
this in ‘Immediate Actions Taken’. If the hazard has been resolved, this should be noted in ‘Suggested
Corrective Actions’ together with your recommendations
Reported By - This is usually the person entering the hazard
Attachments - Attach photos or additional documentation relating to the hazard to assist with the
investigation.
Submit - A successful message will appear. This action will immediately send the hazard report to the WHS
Unit for allocation to the appropriate investigator.
A message will appear confirming the report was submitted successfully (see Figure 03 below).
Note: If a message similar to Figure 03 does not appear and the hazard report is still visible, check to see if any of the
mandatory fields ( * ) need to be filled in. There may be a message such as ‘Required Field’ by each (*) field
that is blank
