CSU Public Portal User Guide Reporting an Incident – Quick Reference The CSU Public Portal is available to all staff, students, contractors and visitors To report an Incident via the CSU Health and Safety Public Portal go to: https://csupublicportal.scrimonline.com.au/Main.aspx# (Figure 01: Log in) Click here to start From the menu bar select ‘Report an Incident’ A form called Incident Report will open (see Figure 02 on the next page). Please complete all sections on the form. Remember a Red Asterisk (*) means the field is compulsory Location Details – Enter the location of where the incident occurred. If the incident is not related to a Faculty or Division or you are unaware of this information then choose the Faculty /Division that you belong to. “‘Further Location Details’ should then be used to help describe the actual site and location in more detail. Incident Details – Enter a brief description of the incident and any actions taken. Please include the task or activity that you were undertaking at the time i.e. walking, having lunch, teaching, etc Reported By - This is the person entering the incident. If you are reporting on another person’s behalf, then the injured person or witness details will be added later (see point 4) Injured Person or Witness– Please be sure to complete all mandatory fields before selecting this link. A new screen called ‘Injured Person or Witness’ will open in front of the ‘Incident Report’(See Figure 03) P a g e | 1 of 5 V1 – 28 04 15 – INC1 CSU Public Portal User Guide (Figure 02: New Incident) Location details refer to the place where the incident happened If you are unsure of the location – choose your own faculty/Division. Use the further details box to add more information about the area. 12hr clock AM/PM Please provide as much detail as possible This is the person entering the incident – may be the injured person, witness or the supervisor Click on here to add the details of the injured person or witness – see next screen shot –Figure 04 Photos or documents can be attached here i.e. certificates, reports, statements etc. P a g e | 2 of 5 V1 – 28 04 15 – INC1 CSU Public Portal User Guide Add an Injured Person or Witness – Part 1 On selecting the ‘Add Injured Person or Witness’ link the following screen will appear (see Figure 04 below). Follow these steps if the you or another person was injured. If the involved person was not injured see over the page. Mandatory - All fields marked with a red asterisk ( * ) must be completed before you can ‘Add an Injured Person’ or ‘Submit’ the form. Injured Person– Complete all mandatory fields marked with an asterisk (*). These are the details of the injured person or witness, where they work and contact information Injury details – If the person was injured, select ‘Yes’. The remaining questions will become active allowing you to fill in the injury details. Fields marked with an asterisk (*) are compulsory. Submit – Select Submit. The form will close and return to the ‘New Incident Report’ where you have the option to add another injured person or witness OR finalise the ‘New Incident Report’ (Figure 03) This information relates to the injured person or witness of the incident – complete mandatory fields By clicking on YES – the field below become active and injury details can be added Describe the treatment and any other information required If the injured person remained at work please click on YES and complete the date and time Clicking on SUBMIT will take you back to the Incident Report screen where you can complete the report P a g e | 3 of 5 V1 – 28 04 15 – INC1 CSU Public Portal User Guide Add an Injured Person or Witness – i.e. NOT injured or witness only – Part 2 Please add contact details for other involved people who were not injured during the incident, for example witnesses or other contacts that may assist the WHS Unit with the investigation (see Figure 04 below). Injured Person or Witness Details – Complete all mandatory fields marked with an asterisk (*). These are the details of the injured person or witness, where they work and contact information Injured – If the person was not injured, select ‘No’. The injury details will be greyed out. Submit – Select Submit. The form will close and return to the ‘Incident Report’ where you may choose to add another injured person or witness OR finalise the ‘New Incident Report’. (Figure 04) Complete all details of injured person or witness here If there is no injury or if you are simply reporting the witness details – select NO Click on SUBMIT and return to the Reporting Screen to complete the report P a g e | 4 of 5 V1 – 28 04 15 – INC1 CSU Public Portal User Guide SUBMITTING YOUR REPORT: Click on Add Attachment to add any photos or documents incl. medical certificates or reports you may have. Ensure you click on Save and click Submit when complete. A message will appear confirming the report was submitted successfully (Figure 05).
