The second annual review for Survey and Manage (S&M) species has been completed in compliance with the
2001 Record of Decision (ROD) for Amendments to the Survey and Manage, Protection Buffer, and other
Mitigation Measures Standards and Guidelines (S&Gs), and the changes recommended in this review are
hereby adopted (Enclosure 1).
The Regional Interagency Executive Committee (RIEC) has reviewed the record of work compiled during the Annual
Species Review including documentation of new information for individual species and notes on panel discussions,
comments, and recommendations and found that the recommended changes for each species are within the expectations
for adaptive response to new information as described in the 2001 ROD for S&M. The Supplemental Information
Report, prepared for the 2002 Annual Species Review, documents that the predicted effects of the S&M mitigation
measure will not significantly change from those described in the Final Supplemental Environmental Impact Statement
(FSEIS), which accompanied the 2001 ROD.
This memorandum changes the category placement for species displayed in Table 1-1 of the 2001 S&M ROD, as
amended June 14, 2002. The specific changes are summarized in the enclosed Results and Implementation Summary
table (Enclosure 1) and are included in the revised Table 1-1 (Enclosure 2). Each change documented for each species
in the Results and Implementation Summary table represents a separate and independent decision even though they are
all being transmitted in this one memorandum correspondence. Implementation requirements for the different
provisions of the S&M Standards and Guidelines are provided below and summarized in Enclosure 1. This decision is
effective today.

Application to Management Activities in the Planning Phase with No Signed National Environmental Policy Act
(NEPA) Decision or Decision Document as of the Effective Date of This Decision
Implementation of Manage Known Site Requirement.
The manage known site requirement is effective immediately in the extended ranges for the 22 species that have a
range extension documented and remain in Categories A, B, C, D, or E. The known site requirement is also effective
immediately for known sites that could be impacted from grazing for what was previously referred to as Monadenia
fidelis salmonensis because of the recent taxonomic revision merging M. f. salmonensis, M. f. klamathica, and M. f.
ochromphalus into the new taxon, Monadenia infumata ochromphalus.
The four species that have been removed in all of their range, solely because they are no longer considered closely
associated with late-successional or old-growth forest, will be considered for inclusion in agency special status species
programs. The requirement to manage known sites will remain in effect throughout their range until their disposition is

clarified in the special status species consideration. Known sites for Cypripedium fasciculatum located in the
Washington Eastern Cascade physiographic province will be removed from S&M mitigation and managed in
accordance with agencies’ special status species policies. For all other species removed from S&M or moved to
Category F, the manage known site requirement is no longer in effect in the removed portion or Category F portion of
their range, and all of these sites are released for other resource considerations.
Implementation of Pre-Disturbance Survey Requirement.
One mollusk species, Monadenia fidelis minor, has moved from Category E (pre-disturbance surveys not required) to
Category A (pre-disturbance surveys required). Ten species (Cryptomastix hendersoni, Deroceras hesperium,
Hemphillia malonei, Megomphix hemphilli, Monadenia chaceana, Monadenia infumata ochromphalus, Oreohelix n. sp,
Pristiloma arcticum crateris, Prophysaon coeruleum, and Plethodon stormi), which currently require pre-disturbance
surveys, have had their ranges extended. Survey protocols for all of these species have already been transmitted to the
field. The survey protocol for Monadenia fidelis minor will apply to all activities throughout the range of the species
for which NEPA decisions or decision documents are signed after September 30, 2004. The survey protocols for the
ten species with range extensions will apply to activities located in the extended ranges whose NEPA decision or
decision document is signed in the calendar quarter following the first full survey season (as defined in the protocol)
from the date of this memorandum. Pre-disturbance surveys (including equivalent-effort surveys) will continue
without interruption in any portion of a species range that was included in a management category that required pre­
disturbance surveys and did not have a category change.
There will be no grace period for conducting pre-disturbance surveys for Pristiloma arcticum crateris that changed
from an equivalent-effort pre-disturbance survey requirement (B2,4) to a practical pre-disturbance survey requirement
(A2 ), in any portion of the species range that previously required equivalent-effort pre-disturbance surveys. Pre­
disturbance surveys will continue to be implemented for this species in accordance with the most current mollusk
survey

protocol without interruption throughout all but the extended portion of the species’ range. The pre-disturbance survey
requirement in the extended portion of the species range will be applied in accordance with the direction provided in
the previous paragraph.
Pre-disturbance surveys will no longer be required in the portion of the range in which a species has been removed
from S&M, including species which have been removed because they are no longer considered closely associated with
late-successional or old-growth forests, and are under review for placement in agency special status species programs.
Pre-disturbance surveys will also not be required for Platismatia lacunosa in the portion of the range that moved to
Category E.
Application to Management Activities with Signed NEPA Decisions or Decision Documents Before the Effective
Date of This Decision
For activities under awarded contract or signed permit, or if actual habitat disturbance has already commenced
using agency crews:
None of the changes from the FY2002 Annual Species Review are applied to these projects.
If activities are not under an awarded contract or signed permit, or actual habitat disturbance by agency crews
has not begun:
Implementation of Manage Known Site Requirement.
Sites of species now requiring management of known sites will be managed as described under Application of Manage
Known Sites Direction under the Timing Requirements for Pre-Disturbance Surveys section on page 24 of the 2001
ROD S&Gs. This applies to the extended ranges for the 22 species that have a range extension documented and
remain in Categories A, B, C, D, or E and known sites for what was formerly considered Monadenia fidelis

salmonensis.

Implementation of Pre-Disturbance Survey Requirement.
Pre-disturbance surveys previously required for those species previously listed as Category A, C, or “equivalent effort”
should have already been completed for projects with signed NEPA decisions or decision documents. If, however,
surveys have not been completed, then pre-disturbance surveys will no longer be required in the portion of the range in
which the species is removed from S&M or moves to Category B, D, E, or F.

No surveys are required for Monadenia fidelis minor that moved from Category E to A as a result of this decision. In
addition, no surveys are required for those species where range changes will result in new areas identified as needing
pre-disturbance surveys.
NEPA Documentation for Habitat Disturbing Activities.
Future agency NEPA decisions or decision documents for habitat disturbing activities should incorporate by reference
the results of the 2002 S&M Annual Species Review to document the changes affected by this memorandum. Refer to
the 2001 ROD S&Gs, pages 18-19; Forest Service November 20, 2001, memorandum, file code 1900/2620; and BLM
Information Bulletin No. OR-2002-033 for further details, (www.or.blm.gov/surveyandmanage/).

Forest. Based on this known site, the south range for P. stormi is now described as follows: The eastern boundary line of the south range
follows interstate highway 5 (I-5) south from the Siskiyou Crest to the northernmost crossing of I-5 and the R7W/R6W boundary line. It
continues south along this line to the Klamath/Trinity National Forest boundary. The southern boundary line of the southern range continues
southwest along the forest boundary to the Scott River/Salmon River District boundary. The western boundary line of the southern range
continues northwest along the Scott River/Salmon River District boundary to the Humboldt/Mt. Diablo Meridian. The western boundary of the
