-Shepherd’s Clinic, Healthcare for the Homeless, TAP, East Baltimore Medical Center and more
Assistance obtaining identification and documentation:
-State ID, Birth Certificates, Social Security Cards and more
Assistance with benefit enrollment:
-MEAP, EUSP, TDAP, TCA, Independence Cards and more
Assistance with Medicare/Medicaid:
-QMB, SLMB, Disability and more
Case management and community outreach:
-Follow-up phone calls and home visits, limited community health outreach worker program
Free preventative health screenings:
-Hypertension, Diabetes, vision, and more
Assistance with outstanding hospital bills

between 12-5.
