Assistance Animal Application Process
The health and safety of Drew University students, faculty and staff are important concerns. Only assistance animals that meet the
criteria detailed in the application form will be exempt from the rules that otherwise restrict or prohibit animals.
A student requesting the use of an assistance animal must complete the application form available on the Undergraduate Housing web
page.

The completed application along with an Assistance Animal Application must be submitted to the Disability Specialist in the
Academic Services Office. This request should be made as soon as possible, preferably prior to the start of the semester to best
enable the scheduling of housing arrangements.
The student must present current documentation of disability including statements from medical provider/diagnostician which
describes the criteria used to assess the impact of the disability.
The student must provide appropriate documentation of their disability that clearly and explicitly explains the need for an
assistance animal and its specific function.
Requests that lack evidence of the animal’s necessity to the student will be denied.
The application form must be completely filled and signed and dated.
Exclusion from Campus
An Assistance Animal may be excluded from the University premises or specific areas of the campus for the following reasons:

The animal’s behavior is disruptive, not controlled and the owner does not take effective corrective action. Disruptive
behavior may include barking, whining, growling, wandering, sniffing people, initiating contact with someone without
permission.
The animal poses a direct threat to the health or safety of others.
Animal illness.
Hygiene- dirty, strong odor, not groomed, evidence of fleas, ticks, etc.
Animal is not on a leach.
Aggressive behavior.
Housing Assignment
If an assistance animal is approved for a student living in University housing, the student will be assigned housing that will seek to
minimize the possibility of other students being negatively impacted by the animal.

I attach my signed and dated health care professional’s documentation of need for assistance. (Document should
specify reason for need).

I have provided a health certification signed by a licensed veterinarian indicating that my animal is up-to-date on all
vaccinations.

I agree to maintain control of my animal at all times.

I agree to accompany my animals or to properly secure it.

I understand that my animal must wear a leash, collar, cape, or other device to secure it and additionally, if not
secured in a cage or other device, must be controlled by verbal commands.

I agree my animal will wear appropriate visible identification that identifies in writing that the animal is a service
animal.

I agree my animal will be licensed and will wear a valid vaccination tag at all times.

I understand my animal must be house broken, well-groomed, odor free, and not infected with external parasites, i.e.
ticks, fleas or lice.

I agree that I am responsible for the sanitary disposal of my animal’s waste while on campus.

I agree that my animal will not be in facilities outside my residence, including areas where food is being prepared,
research labs or areas requiring protective clothing.

I agree that I am responsible for any damage or personal injury caused by my animal and further agree to pay the
University any costs associated with clean up or otherwise remedy any damage caused by my animal.

I understand that others may be impacted by the presence of my animal in a residential setting. As a result, I
understand and agree that I or the other person may have to be relocated at the discretion of the University after
consultation with impacted parties.

I understand that I must comply with all the requirements and agree to the restrictions as outlined in the Assistance
Animal Policy and/or in this application.
