Community Outreach And Linkage To Care For HIV Infected Minority Population
As part of the Minority AIDS Initiative, local service organizations provide outreach and education to minority communities; help people with HIV
to enroll in programs that pay for HIV medications, medical care and health insurance premiums; and connect them with medical and supportive
service providers for a full range of HIV/AIDS care. The organizations provide in-person assistance to enroll in the HIV Uninsured Care
Programs (ADAP/ADAP Plus/APIC) as well as more complete health coverage programs such as Medicaid, Medicare, or private insurance
through the Marketplace/Exchange: New York State of Health. The ADAP Plus Insurance Continuation (APIC) program can pay the insurance
premiums for cost-effective insurance policies, including COBRA continuation coverage for people who lost their jobs or those policies
purchased through the New York State of Health.

Health education and risk reduction education and/or counseling services are provided to individuals and/or groups and range from targeted
outreach to prevention counseling to group interventions to broad community level interventions (CLI). Individual-level interventions (ILI)
provide individualized health communication, health education, and risk reduction counseling to assist clients in creating plans for individual
behavior change and making ongoing appraisals of their own behavior. These interventions may consist of one or multiple sessions.
Group-level interventions (GLI) may include peer education and support to promote and reinforce safer behaviors. Prevention interventions

Legal supportive services programs provide assistance for HIV affected families with dependent children to make decisions focused on future
care and custody plans. Programs help HIV positive parents identify potential caregivers or guardians for their dependent children, help with

Health Homes provide comprehensive care management to high need Medicaid recipients with chronic illnesses, including persons living with
HIV. HIV Care Management agencies provide access to, coordination, and integration of physical, behavioral, and community support services

HIV/STD/HCV Prevention Education/Risk Reduction Services
Health education and risk reduction education and/or counseling services are provided to individuals and/or groups and range from targeted
outreach to prevention counseling to group interventions to broad community level interventions (CLI). Individual-level interventions (ILI)
provide individualized health communication, health education, and risk reduction counseling to assist clients in creating plans for individual
behavior change and making ongoing appraisals of their own behavior. These interventions may consist of one or multiple sessions.
Group-level interventions (GLI) may include peer education and support to promote and reinforce safer behaviors. Prevention interventions
delivered to groups may be single or multiple sessions, and often provide interpersonal skills training to negotiate and sustain appropriate
behavior change. CLIs seek to reduce risk behaviors by changing attitudes, norms, and practices of community members, and may include a
combination of intervention types (e.g., outreach, health communication, public information) or units of delivery (e.g., individual, group,
community)

Legal Supportive Services For Families
Legal supportive services programs provide assistance for HIV affected families with dependent children to make decisions focused on future
care and custody plans. Programs help HIV positive parents identify potential caregivers or guardians for their dependent children, help with
disclosure of HIV status, grief and bereavement services, transitional services to stabilize newly blended families following parental
incapacitation as well as short term supportive counseling to assist families with these issues. Legal supportive providers work directly with
legal service providers to ensure families can make their care and custody plan final and address any legal barriers during the future care and
custody planning process.

Syringe Exchange
Provides new, sterile syringes without a prescription to active injection drug users, steroid and hormone users and others who inject substances
to prevent transmission or acquisition of blood borne diseases. Provision of syringes includes the offer of short counseling on various safer
