Template for Priority Action Plan
a)
Define scope by health problems to be addressed.
– Where are biggest disease problems, biggest disparities?
b) Identify evidence based, promising and next policies, programs, and
practices for action that address five levels described in Frieden pyramid.
c) Assure that policies, programs and practices for action selected in each
priority area address cross cutting issues where possible:
– Access to quality health
- Life course perspective
– Health disparities
- Social Determinants of Health
– Gender perspective
- Oral Health
d) Identify key sectors/organizations that need to take action on each
intervention selected
e) Propose timetable and milestones for implementation over five yrs.
f) Select indicators to set reachable targets by 2017 and monitor progress
at identified time intervals.
