Notice: In response to a number of requests, the MPH Practicum Awards have been expanded to include a
group project category. Award application deadline (for group and individual awards) has been extended
to March 11, 2015.
Practicum Awards for MPH Students
The Office of Public Health Practice and Training, in partnership with the MPH Program Office, recognizes
MPH students for public health practice contributions made through their practicum experiences. Up to 10
individual projects and one group project will be awarded.
MPH Practicum Awards are self-nominated, but require a letter of support from the preceptor/supervisor. To
be considered, you should submit the following information to [email protected]:
1. Statement (up to one page single-spaced, 1-inch margins, 12-point font) that describes the following:
 Title of Practicum Project
 Practicum experience activities completed
 Project impact assessment (e.g., implications for policy, practice, future programming, etc.)
 Self-reflection (for group submissions, please share how the project enriched the
group's MPH experience):
o How the project enriched your MPH experience
o How the project impacted your future career goals and objectives
2. Resume/CV (for group submissions, please provide a resume/CV for each student in the group)
Note: The above documents should all be submitted as attachments to a single email.
3. In addition, please ask for a letter of support from your preceptor/supervisor which should be
emailed separately to [email protected] with the subject heading “Student(s) Last Name(s) –
Practicum Awards Support Letter”. One additional optional letter of support from an individual
impacted by the project (e.g., stakeholder, community member, etc.) could also be requested.
Deadline for nominations is Wednesday, March 11, 2015 at
11:59 p.m.

Note: Both full-time and part-time MPH students are eligible. Practicum projects finishing up shortly after the
submission deadline are still eligible for the award.
