A Q&A session will follow the demo. 11:15 Break 11:30 Conversation with DOH Leadership In this session, the DOH will present and discuss newly released health home performance data that provides insights in to health homes’ outreach and engagement efforts to date. An audience Q&A session will follow the presentation. 12:30 p.m. Networking Lunch 1:15 PCDC Integrated Care Planning Initiative Update Karla Silverman will provide a brief update on PCDC’s Integrated Care Planning Initiative and related efforts to support collaboration and integrated care planning processes between clinicians, social and behavioral health providers and care management staff. The group will then hear from a panel comprised of representatives from HealthFirst, FEGS, and Brooklyn Health Home, who will discuss how they are collaborating to provide care coordination for HARP-eligible health home enrollees. Following the state update and panel, participants will discuss reactions to the models presented and considerations for key areas of focus moving forward. 2:15 Changeover from Direct Billing: Lessons Learned and Best Practices This session will focus on the upcoming change away from direct billing for legacy services.
