Rachel Davis will review the day’s agenda, provide an update on
learning collaborative activities, and describe the goals for the
meeting.

Bruce Nisbet will start the meeting off with a presentation on a recent
success his health home has experienced.

MAPP Rollout and Demo
Chip Barnes and Laura Moretti will provide an in-depth demo of the
portal to highlight features and functionality. A Q&A session will
follow the demo.

Conversation with DOH Leadership
In this session, the DOH will present and discuss newly released
health home performance data that provides insights in to health
homes’ outreach and engagement efforts to date. An audience Q&A
session will follow the presentation.

PCDC Integrated Care Planning Initiative Update
Karla Silverman will provide a brief update on PCDC’s Integrated
Care Planning Initiative and related efforts to support collaboration
and integrated care planning processes between clinicians, social and
behavioral health providers and care management staff.

HARP Update
The DOH and OMH will begin this session with an update on the
BHO/HARP transition. The group will then hear from a panel
comprised of representatives from HealthFirst, FEGS, and Brooklyn
Health Home, who will discuss how they are collaborating to provide
care coordination for HARP-eligible health home enrollees. Following
the state update and panel, participants will discuss reactions to the
models presented and considerations for key areas of focus moving
forward.

Changeover from Direct Billing: Lessons Learned and Best
Practices
This session will focus on the upcoming change away from direct
billing for legacy services. Audience members will be encouraged to
share strategies for success and lessons learned to date for creating
effective billing structures between downstream providers, health
homes and plans.
