13, 2013
The Department is currently reviewing the data.
◦ If there are issues with your data submission the Department
will be contacting you to resolve them.


FACT GP / HH Functional Assessment Data should

Does the member need to sign the Health Home Consent form
in order to share HH CMART and assessment data with the
Health Home?
◦ As long as the CM agency and the Health Home have a signed contracted
agreement in place, a member does not need to sign the Health Home
consent for the CM agency to share the HH CMART data with the Health
Home.
◦ Health Home members are considered passively enrolled on all DOH
assignment lists. The member is considered actively enrolled when they
are found, and agree to receive HH Services or participate in the HH
program.
◦ Consent should be obtained as soon as possible. But you can care
manage them as long as you can without sharing member data with the
care management team. But it’s a program decision as to how long you
can care manage the member without obtaining that consent.

When will the Health Home consent be available in
other languages?
◦ The Department has been working on incorporating
language in the Health Home consent that will allow access
to the OMH PSYCKES system and allow sharing of the NYSID
number.
◦ Once the language changes in the consent are approved by
legal, and then the cost of translating them is approved,
the Department will provide translations of the consent in
other languages.

Data for ALL elements in the HH-CMART tool
This will include all data for the first two quarters of
2013 (January – June 2013)

How to Submit
Lead Health Homes will send data on all members in
the HH CMART tool using the HCS Secure File Transfer
application to ‘Laura Morris’

Q: If someone had an initial and a discharge FACT-GP completed
in the same reporting period, how should this be recorded?
A: The program should report the most recent assessment date
to the department. ( Please send feedback – we can look at this
again if programs believe this will be a frequent issue)

Q: Would internet searches for the member (i.e. Google search or
reverse look-up to get phone numbers or correct address) be
counted as attempts in the Outreach effort field?
A: Yes. If found, the member should be contacted. Finding the
phone number and calling would be counted only once in this
scenario. Attempts to outreach should be varied and increase in
degree of ‘difficulty of task’ with each unsuccessful attempt.

Q: What if a client has a Managed Care Organization
that is not in the PlanId drop down list (Field #1)?
A: The Health Home CMART allows manual entry in
fields. If a PlanId is not in the drop down list, enter
it manually. Contact the Health Home Program via
email to let us know what is missing.
*Reminder: uploading data into the HH CMART tool
will wipe out any manual entries, so save those for
last.
