New York State Hospitals’ Community Service Plan Summaries 2014 Introduction: The New York State Department of Health developed summaries of the Community Service Plans submitted by hospitals in November 2013. Each report was reviewed by at least two reviewers. Reviewers noted priorities, goals and interventions identified in the reports. Their observations and comments were consolidated into this summary.
