Introduction: The New York State Department of Health developed summaries of the
Community Health Improvement Plans based on its review of the Community Health
Assessments and Community Health Improvement Plans submitted by local health departments in
November 2013. Each report was reviewed by at least two reviewers. Reviewers noted priorities,
goals and interventions identified in the reports. Their observations and comments were
consolidated into this summary. This summary was provided to local health departments in
conjunction with the year one Prevention Agenda 2013-2017 update survey for feedback and
corrections which were then incorporated into this updated report.
