New York is pleased to announce the submission of its application for Round 2 Model Test
funding under the State Innovations Model funding opportunity announcement (FOA). On
Friday, July 18, New York submitted its application with over 30 letters of support. Our
application would not have been possible without the input and guidance of numerous
stakeholders. Thank you for your support.
Attached you will find New York’s submitted Project Narrative; this will also be posted to our
DOH website. The proposal requirements and materials associated with this FOA can be
accessed on CMMI's SIM webpage, found here.
Applications were due to CMMI on July 21. Awards are to be announced in the fall, and the
performance period would begin January 1, 2015.
