Introduction and Address Label
Consumer and Provider Materials Order Form
The AIDS Institute is pleased to offer a variety of educational materials on HIV, sexually
transmitted diseases (STDs) and viral hepatitis for consumers and health and human providers.
The AIDS Institute strives to ensure that educational materials address important topics, are
culturally relevant, available in multiple languages and employ principles of health literacy. All
consumer education materials were developed with input from consumers in the form of focus
groups, review by the AIDS Institute’s Consumer Advisory Committee or other methods of
engaging the specific target population for the specific product. Materials are available in a
variety of formats as indicated on the order form. Some materials are conveniently available in
PDF format for you to print and copy at your own leisure.
All materials are available free of charge. Orders for bulk quantities may be placed using this
Order Form. You are encouraged to download and copy materials instead of ordering printed
copies whenever possible.
How to Use the Order Form: This form is a PDF fillable PDF form, which means you can insert
checkmarks or enter text only in the fillable pieces of this form. Please check off the number of
copies of the material you are requesting and submit the form following the directions listed below.

Is there someone familiar with these materials to talk to if I need help determining which
materials will best meet the needs of my community or address a specific topic?
Yes. Staff from the Educational Materials Initiative are available during regular business hours

What is the procedure for ordering materials?
Please fill out your address on page 1, and then use the rest of the order form to order the titles
and quantities of the materials you would like. Orders can be submitted to

What is the cost of these materials?
Materials are provided to New York State providers, consumers, and community members free
of charge.
How are orders delivered?
 Orders are shipped from the New York State Department of Health Distribution Center in
Albany. They cannot be picked up in person.
 Orders cannot be sent to Post Office Boxes. You will need a street mailing address.
How long do orders take to be delivered?
Please allow 4-6 weeks for delivery.
What happens if a publication is temporarily out of stock?
Publications that are temporarily out of stock will be noted on a packing slip when you receive
the order. If you have any questions about the availability of publications, please email

What is the fax number to fax my order form in?
