1. Student’s Financial Aid file must be complete
IE: FAFSA, Verification (if required), High School Documentation, Data Form Submitted
2. Student must be eligible for federal student aid.
3. Parent must complete Parent Plus Entrance, Parent Plus Application and E-MPN at www.StudentLoans.gov .

4. Parent must submit the Direct Parent PLUS Loan Request Form to the Financial Aid Office. 10 days are required
to process this loan request.

This application is subject to credit approval by the US Department of Education. Credit denial may be appealed, call Direct
Loan Servicing at 1.800.557.7394
Student must submit FAFSA, complete required steps and submit requested documentation to the Financial Aid office.
Failure of the student to remain enrolled in at least 6 credit hours may result in cancellation of this loan.
Student must maintain Satisfactory Academic Progress as established by NCSC and obtain a degree/LPN certificate within
150% of the minimum credit hours required.
This Direct Parent PLUS Loan award may be adjusted when/if additional financial aid is applied to the student account.
Your submission of this form authorizes the US Department of Education to initiate a credit check. Your submission of this form also
authorizes NCSC to process this application electronically (EFT), proceeds from this loan will be applied to the student account. Any
possible refund will be issued to the student.
