• JMHCP is a cross systems collaboration with criminal justice and
behavioral health for the purposes of reducing recidivism and
supporting recovery
• JMHCP Technical Assistance includes training and consultation
organized by Sequential Intercept Points
• Community Care Coordination – A program that crosses all
intercept points, which ensures access to high quality services and
promotes recovery using a Care Coordinator who creates and
supports implementation of Individualized Service Plans

Affordable Care Act and NYS Medicaid Redesign
