Asthma Action Plan
The purpose of this Asthma Action Plan is to help families become proactive and anticipatory with respect to asthma
exacerbations and their control. The Asthma Action Plan should be used as an education and communication tool between the
provider and the patient and his or her family. The patient/family should be able to demonstrate an understanding of the plan
and the appropriate use of medicines.

Have Asthma? Take a Look Around You
Designed to help people with asthma identify their triggers and to suggest practical ideas for reducing exposure to triggers
indoors and outdoors. This brochure was developed by clinical and environmental health professionals with input from
literacy teachers and students. Its easy-reading presentation includes lots of pictures and stories about steps to take to
reduce asthma episodes. It also includes a two-sided, tear-off panel for patients to use with their doctors to better manage
asthma.
