Select age group 8 - 14
Select level of play. Indicate divisional brackets typically played or age group such as U14 A or U14 B, etc.
Indicate level of play within your organization and within your state Tiers or Leagues. If your league uses color
designation please clarify level of play. Or indicate minor or major level.
Submit one registration form per team with payment of $ 850 for each team. If multiple teams are entered for
an organization, you can submit one check for all teams.

Canadian teams must submit travel permits.
Your association will be financially responsible for the tournament cost on a team basis. The fee for this tournament is
$ 850 per team to participate in this tournament. Any association withdrawing a team(s) after the tournament Director has
made ice arrangements for the tournament must still pay for the team(s) fees for this tournament. All ice arrangements
will be completed by February 1, 2016. Team withdrawals prior to February 1, 2016 will be required to pay a $100
administrative fee for all teams withdrawing from this tournament. NO refunds after February 1, 2016.
Registrations are limited – sign up as early as possible.
