Modification to the “Response to Questions” Post Date
The New York State Department of Health has extended the Assisted Living Program 3400
Initiative “Response to Questions” post date to May 22, 2015 (on or about).
Modification to the Application Due Date
The New York State Department of Health has extended the Assisted Living Program 3400
Initiative submission deadline to June 22, 2015, no later than 3 PM. Applicants are encouraged
to submit at least several days prior to the deadline. Late applications will not be accepted.
