Term of Contract (Page 17-18) has been amended and changes are reflected in bold type below. It now reads as follows: C. Term of Contract All contracts resulting from this RFA will be effective only upon approval by the New York State Office of the Comptroller. It is expected that contracts resulting from this RFA will have the following time period: April 1, 2015 to March 31 October 1, 2015 – March 31, 2021, with annual extensions deemed necessary and appropriate by the Commissioner and approved by the Director of the Division of Budget. Continued funding throughout this period is contingent upon availability of funding and state budget appropriations. DOH also reserves the right to revise the award amount as necessary due to changes in the availability of funding. Clarifications 2. All Applications must be submitted via electronic means. The Application(s) must be emailed to [email protected] on or before May 6, 2015. There is no specific time by which applications must be received by on May 6, 2015. 3. The Letter of Attestation is requested for applications identical to those submitted under RFA # 1410100351. 4. All Applications submitted under RFA # 14-10100351 have been rescinded. An applicant with a project originally submitted under RFA # 14-10100351 must re-submit projects through the appropriate application process (A or B) as described on page 9 of the RFA. 5. The Department requests applications submitted under RFA # 14-10100351 now submitted under RFA # 15-04100252 include forms and attachments that reference the new RFA number. However, references to either RFA # are acceptable. 6. The PPS leads should use Attachment 1, referencing the new RFA number 15-04100252, to rank their participants projects, and be sure to include any new applications that might be received by the PPS lead. The process by which the PPS lead receives, ranks, and submits the applications is determined by the PPS. 7. All Not-For-Profits must have Prequalified Status in Grants Gateway on May 6, 2015 to be eligible for funding. 8. For purposes of this grant opportunity, New York City is defined as the counties of Bronx, Kings, New York, Queens and Richmond. 9. Allowed costs for projects pertaining to the Capital Restructuring Financing Grant application will be those incurred on or after the anticipated contract start date, as amended above. 10. Please refer to the Q&A’s that are posted at the CRFP webpage for all questions, including those regarding eligible projects, organization eligibility, and the process by which to apply. a. Answers referring to the application’s submission due date should now read May 6, 2015; and b. Answers referring to a statewide distributions of funds should now be replaced with “For purposes of determining regional proportionality, project applications will be designated as either “New York City” (NYC) or “Rest of State” (ROS) and such designation shall refer to the physical location of the project for which funding is being requested. NYC is defined as the counties of Bronx, Kings, New York, Queens and Richmond.
