2016 TUITION OPTIONS AND CREDIT CARD FORM

TUITION OPTIONS—Please check the one that applies:
___ 2-week residential student = $2040 (tuition/room & board/recreation)
___ 2-week residential student + secondary (minor) area of study = $2,270 (tuition/room &
board/recreation)
___ 2-week commuter = $1,540 (tuition/lunch & dinner, recreation)
___ 2-week commuter + secondary (minor) area of study = $1,770 (tuition/lunch & dinner, recreation)
*Lunch is not included in the above listed costs for two meals that the students eat off-campus.
PAYMENT DUE DATES
*Nonrefundable Application Fee: Payment by check only: $75 by March 15th, 2016; $100 after March
15th, 2016 (Application fee is in addition to tuition, room & board.)
*Nonrefundable Deposit: Due 7 days after acceptance. Payment by check: $500. Students who wish to
pay their nonrefundable deposit by credit card must submit their entire tuition payment by credit card
at this time. (Credit card payments are assessed a 3% fee.)
*Balance: Payment by check only, due May 1, 2016 (Applications received after May 1st must pay tuition
in full within 7 days of acceptance.)
*Damage Policy: Please complete the credit card information below, even if you are paying tuition by
check. Should there be any damages that are assessed to your students’ account at the end of the
Academy, your credit card will be billed along with a 3% fee.
