IMPORTANT NOTES:

If your internship spans multiple quarters, you must re-submit these forms each quarter.

You must attach to these forms an official internship description on company letterhead, as
drafted by your internship supervisor.

During the internship, the student will participate in a closed-membership online discussion
forum with other students. At the conclusion of the internship, students will complete a selfreflexive report. This report, along with an evaluation completed by their internship supervisor,
will determine the student’s grade.
Internship Site Requirements
The internship hosting organization should provide supervision and mentoring to the student, as well as offer
consistent feedback to the student with the understanding that the internship is foremost an educational
experience. The internship supervisor should, to the best of their ability, provide the student with an array of
projects with which the student may develop and practice skills related to their professional field. At the end
of the internship, the supervisor will be asked to fill out an evaluation of the student’s performance.
