Purpose of this form
According to the Code of Ethics of the National Association of Schools of Music (NASM), you may be considered eligible
for financial aid during your initial quarter of enrollment at the Lamont School of Music only if the chief music executive
(e.g., dean or music department chairperson) of your current school specifically approves. In accordance with this code,
please complete Part I below and then submit this form to the music executive at your school. You cannot be considered for
financial assistance or scholarship at the Lamont School of Music until we receive this form.
