In 2015, you and your covered spouse are each eligible to receive a well-being reward of up to 480
well-being incentive credits if you are enrolled in any of the BCBS or UHC health plan options and
complete the activities below between January 15, 2015 and December 15, 2015. That’s a family
total of 960 well-being incentive credits.
What to DO
1.
ASSESS YOUR HEALTH
Complete your 2015 Healthways Well-Being Assessment®
(WBA), a confidential, online questionnaire that will take about
20 minutes.
KNOW YOUR NUMBERS
Complete a 2015 biometric screening and submit results (body
mass index, blood pressure, cholesterol, glucose).
The biometric screening must be completed at a SHBPsponsored screening event or by your physician and your
results submitted appropriately on the 2015 Physician
Screening Form.

TAKE ACTION
It’s your choice! Complete the coaching pathway, online
pathway, or a combination of both.
COACHING PATHWAY
• Complete your WBA and,
• Actively engage in telephonic coaching.
ONLINE PATHWAY
• Complete your WBA and,
• Record five online activities using the same tracker within
four consecutive weeks and earn 40 well-being incentive
credits. You can earn these rewards up to six times. A
sample online activity would be to track your daily steps
five times within four consecutive weeks.

If you remain in a BCBS HRA option (Bronze, Silver, Gold) you will have access to any well-being HRA dollars earned in 2014.

The Be Well SHBP well-being program offers a holistic approach to your personal
well-being. By providing some of the most effective health resources and coaching
services available, you and your covered spouse will be empowered to achieve your
personal or family well-being goals and pursue a longer, healthier, and more fulfilled life.
THE TYPE OF SUPPORT YOU RECEIVE.
BIOMETRIC SCREENING
• Access to private screening events with a health care professional that offers a
current view of your health and determines if you are at risk for developing certain
conditions.
WELL-BEING ASSESSMENT
• A confidential online questionnaire that assesses your physical and emotional
health and how your lifestyle habits affect your overall well-being.
WEB-BASED PLAN FOR WELL-BEING
• A personalized summary of your overall well-being that offers insight into
actionable steps you can take to improve your health.
• Tailored recommendations, focus areas and a personal dashboard and interactive
trackers to help keep you on the right path.
• Online fitness, nutrition and stress management plans that promote healthy
behaviors to help you reach your healthy best.
PERSONAL WELL-BEING TELEPHONIC COACHING
• A series of one-on-one telephonic coaching sessions with your personal well-being
coach who can help you to set and keep you on track to achieve your health goals.
• Work on your weight, manage your stress, level of exercise, family nutrition, quit
using tobacco or better manage a health condition.
• Each coaching program will be created and personalized on your unique needs and
goals.
RESOURCES FOR QUITTING TOBACCO
• Access to QuitNet®, the world’s largest online network of those who have quit or
are quitting.
• Telephonic coaching sessions with a trained counselor.
• Access to Nicotine Replacement Therapy coverage.
GROUPS AND CHALLENGES
• Lets you participate in a community of other people who share similar goals.
• Get support and encouragement in a group way to help inspire you to stick with it
and achieve your goals.
ONSITE WELL-BEING SUPPORT
• Presentations and demonstrations given at SHBP employer worksites on a variety
of topics including healthy eating, family well-being, increasing physical activity,
stress management, preventive care and more.
• Worksite Well-Being ambassadors program to keep you informed and motivated.
