By providing some of the most effective health resources and coaching services available, you and your covered spouse will be empowered to achieve your personal or family wellness goals and pursue a longer, healthier, and more fulfilled life. BIOMETRIC SCREENING • Access to private screening events with a health care professional that offers a current view of your health and determines if you are at risk for developing certain conditions. WELL-BEING ASSESSMENT • A confidential online questionnaire that assesses your physical and emotional health and how your lifestyle habits affect your overall well-being WEB-BASED PLAN FOR WELL-BEING • A personalized summary of your overall well-being that offers insight into actionable steps you can take to improve your health • Tailored recommendations, focus areas and a personal dashboard and interactive trackers to help keep you on the right path • Online fitness, nutrition and stress management plans that promote healthy behaviors to help you reach your healthy best PHONE PERSONAL HEALTH COACHING • A series of coaching sessions with health coaches to guide you to better well-being • Personalized support for a variety of risks and conditions that may affect your health • Provides support if you are currently living with a health condition RESOURCES FOR QUITTING TOBACCO • Access to QuitNet®, the world’s largest online network of those who have quit or are quitting • Phone coaching sessions with a trained counselor • E-mail tips offering motivation and encouragement • Access to Nicotine Replacement Therapy coverage GROUPS AND CHALLENGES • Let you participate in a community of other people who share similar goals. Don’t forget, benefit covered spouses are also able to earn HRA dollars and are eligible for rewards! Enjoy the rewards of better health! Starting this January, SHBP members and covered spouses can each earn up to $480 in HRA funds just for participating and engaging in healthy activities that is a family total of up to $960. HRA funds can be used for eligible health care expenses with unused dollars rolling over to the next Plan Year if you keep HRA coverage. The new 2014 action-based incentives will be earned as the action is completed and deposited in the HRA account monthly. To earn these funds, complete the following between January and December 2014: What to DO 1. 2. 3. ASSESS YOUR HEALTH Complete your Healthways Well-Being Assessment® (WBA), a confidential, online questionnaire that will take about 20 minutes. KNOW YOUR NUMBERS Complete a biometric screening and submit results (body mass index, blood pressure, cholesterol, glucose). The biometric screening must be completed at an SHBP sponsored screening event or by your physician or other providers identified by SHBP in published materials. What you EARN Complete BOTH and earn $240 in HRA fund contributions. (WBA must be completed prior to HRA fund distribution.) TAKE ACTION It’s your choice! Complete the coaching or online pathway or a combination of both. COACHING PATHWAY Create your Well-Being Plan. Actively engage in telephonic coaching. ONLINE PATHWAY Create your Well-Being Plan. Record 5 online well-being activities using the same tracker in a month and earn $40 HRA funds. You can earn this for up to 6 months. Sample activities: track exercise five times, record daily steps five times, track food servings five times. Earn up to $240 in HRA fund contributions. (WBA must be completed prior to HRA fund distribution.) Remember, members and covered spouses who met the 2013 Wellness Requirement will receive their $240 HRA incentive fund contribution in January 2014. This is in addition to the $480 HRA dollars each member and spouse can earn in 2014. *The 2014 action-based incentives do not apply to the Medicare Advantage Options. WHAT IS “WELL-BEING”?
