Pharmacy Christian Missions Fund Application

The purpose of the Swanson Missions Fund is to provide financial assistance to Campbell University Christian
pharmacy students who seek to complete a P-4 rotation elective in Pharmacy Christian Missions or who plan
to complete a short term medical/pharmacy Christian missions trip.
Students applying for funds from this account must complete the answers to the questions noted below. Once
completed, students will then e-mail their request for funding to Dr. James A. Boyd ([email protected]).

2. What is the purpose of your mission trip/rotation?
3. What are the dates of the trip?
4. Who is going with you and who will be leading the trip?
5. Are there any other CU pharmacy students going with you? If so, please identify that/those person(s).
6. What will you be doing on the trip? Please provide a description of the activities that will be carried out
on your trip.
7. What is the total cost of the trip for you?
8. How much of the cost of the trip will paid out of your own personal funds and what funds have been
committed to you for this trip to date?
9. What other sources (organization, individuals, your church, etc.) of funding will you be seeking? What
funds have been committed from these sources?
10. Describe how you became a Christian.
11. What church do you attend? Are you a member of this church?
12. What ministries have you participated in at this church?
13. Over the past year, what % of the Sunday services have you attended?
14. Please briefly describe your interest in Christian missions work and provide a brief description of any
previous missions work you have completed.

for you.
16. Will you agree to write up a description of your trip and present it to students enrolled in next year’s
Pharmacy Christian Missions elective?
