•Fill in the required

•Fax the form to the
OASIS Security Team
•There will be email
notification when the
access is ready

•In PeopleSoft, access is granted
by giving a person one or more
roles
•For now, work with your data
trustee on what roles are
appropriate to request
•Be descriptive in the comments
section
•Your signature and your
supervisor’s signature are required
•The OASIS security team will
route the form for data trustee
approval
•Customers will be notified by
email when access has been
granted
