___ Enroll in at least six (6) credit hours for each term of the loan period
___ Meet all standards of Satisfactory Academic Progress (see SUSLA website for policy)

___ Complete Entrance Counseling at www.studentloans.gov (First-Time Borrowers only)
___ ALL STUDENTS must Complete the Master Promissory Note(MPN) at: www.studentloans.gov
Applicant Certification: My signature below certifies that I understand that this is a loan request form, not a promissory note. If I am a firsttime Direct Loan borrower or if I borrowed prior to 2005–2006, I must fill out, sign, and return the actual Master Promissory Note (MPN). If
I have a multi-year MPN on file with the U.S. Department of Education, I am not required to sign a new MPN. Further, I understand that my
eligibility for Federal Direct Loans will be determined by the Office of Financial Aid based on Federal eligibility and need analysis
requirements. Should differences exist among your loan request information, loan related documents, and or the FAFSA, your signature
below gives the Financial Aid Office permission to make adjustments electronically to your FAFSA application and/or loan application.
