Your web application has been marked complete. It will be processed by the Admissions Office when the
admission documents have been received. Please print and complete this page, sign, date, and mail to:
Southern University Shreveport – Admissions Office 3050 Martin Luther King Jr. Drive –
Shreveport, LA 71107. Please include the appropriate application fee ($20, or $30 for international
applicants). First-time freshmen must submit an official high school transcript (we will accept 6th, 7th, or
8th semester transcript), and ACT/SAT scores. Note: First-time freshmen may fax the high school
transcript and test scores to 318-670-6483. However, the final high school transcript must be mailed
from the school.
All transfer students must submit official college transcripts from all schools attended; even if the credits
appear on another transcript.
All former students who have enrolled at another school since last enrolled at SUSLA must submit an
official transcript from the other school.
Please note that all correspondence regarding the status of your application will be sent to the email
address on your application. It is your responsibility to notify us of any changes to your email or
mailing addresses. Your official acceptance letter will be sent to your mailing address. Do not
submit multiple applications, as it delays processing. If there are changes to your application, email
us at [email protected].
