In order to participate in the William D. Ford Direct Loan Program, you must do the following:
NEW STUDENTS or FIRST TIME Federal Direct Loan borrowers at Campbell:
Sign in at www.StudentLoans.gov (using student’s FAFSA user name and password)
• Complete the electronic Direct Loan Sub/Unsub Entrance Counseling
• Complete the electronic Direct Loan Master Promissory Note (MPN) (be sure to click “submit”)

ALL Direct Loan borrowers:
• Complete this Loan Authorization Form and submit to the Financial Aid Office
(Loans must be processed prior to the end of the semester)
• Register in six (6) or more credit hours each semester/block by census date
(Loans will not disburse if your enrollment falls below 6 credits)
• Must meet SAP (Satisfactory Academic Progress) requirements (see website for SAP Policy)
(Loans will not disburse if you do not meet SAP per term of the loan)
Reference your Financial Aid awards for your loan eligibility. If you have been awarded for only one
semester, or will be graduating, indicate the appropriate response for the loan period below.

By my signature, I hereby authorize Campbell University to process my student loans and when the funds become available, to transfer the proceeds of each of my
student loan disbursements for the William D. Ford Direct Loan(s) to my student account at Campbell University. I understand that my student account will reflect a
credit each time a transfer of loan proceeds is made and that the Campbell University Business Office will notify me of such crediting. I understand that my William D.
Ford Direct Loans will enter repayment upon the expiration of my grace period, which begins six months after I graduate, withdraw, or otherwise drop below halftime enrollment. I also understand that the Department of Education may deduct fees before my loan proceeds are disbursed to my Campbell account.
