Dear UB Parent/Guardian:
Your student has been selected to participate in the following field trip:
LOCATION: Family Fit Day, Houston, TX.
DATES: Saturday, April 12, 2014.
Transportation leaves: Sharpstown 8:30am. Lee High School at 8:45am
Transportation returns to: Lee High School 5:15pm Sharpstown at 5:30pm.
Meals will be provided to students.
OTHER IMPORTANT INFORMATION: Please sign the permission slip below and return it to the Upward Bound Office by
Thursday, April 3, 2014 to reserve your spot.
STUDENTS: Please report to Upward Bound room no later than 8:25am.
Please call Ms. Roberson (281)904-5274 or Mr. Spears (405)837-5826 if you have any questions or concerns. You can return
the permission slip to the Upward Bound office.

FIELD TRIP CONDUCT CONTRACT & DRESS CODE POLICY
Behavior & Conduct Standards
When a student and their parent(s) accept the invitation to attend an UB Field Trip, they both agree to follow the rules and
regulations of the program by signing the Field Trip Conduct Contract. By signing this contract, you are agreeing that you will abide
by the behavior standards & dress code policy. Appropriate disciplinary action and parents’ notification will be the consequences of
any infraction of these rules.

Dishonesty (stealing, taking items that do not belong to you, lying, etc.) is not tolerated
Profanity is not tolerated
Alcoholic beverages, illegal drugs, tobacco products and cigarettes are not permitted at any time
PDA (Public display of affection) is not permitted
Any type of sexual activity is inappropriate and not permitted
Practical jokes and/or pranks are not appropriate behaviors at any time
Possession of weapons (guns, knives, karate weapons, etc.) or fireworks are not permitted
Students will be held responsible for any unauthorized phone charges, damages to furniture, fixtures, university
property, equipment, or buses
9. Fighting and failure to follow staff instructions are not allowed at any time
10. Disruptive behavior or defying the authority of ETS staff or volunteers is not tolerated
Dress Code
Shoes and shirts along with other usual clothing articles are required at all times. Sleepwear and indecently torn clothing are not
appropriate for either males or females. Shirts, hats or articles of clothing with profanity, other inappropriate words/phrases, or
tobacco/drugs paraphernalia printed on them are not permitted.
Girls: Crop tops, halter-tops, tube tops, see-through clothing, and short skirts, along with any other revealing articles of clothing are
not appropriate.
Boys: Tank tops are not appropriate.
Students who do not meet the above dress code requirements must wear program issued attire for the day.

Son/daughter to abide by the above mentioned behavior standards (#1-#10) will result in immediate dismissal from the UB
program. I understand that I will be notified immediately and that I will be responsible for traveling to the field trip location to pick up
my son/daughter. I understand that disciplinary action will be taken on students whose parents do not pick up their son/daughter.
