CONTRACT FOR ADMISSION
The Student Success Program (SSP) at Guilford provides extra support and guidance to students. As a student in the SSP
program, you will be given a Student Success Mentor (SSM) to encourage and guide you through your adjustment to Guilford.
We developed the requirements listed below to promote actions that help make students successful.
1.
I understand that my enrollment at Guilford College is contingent upon my full participation in the Student Success Program and if I do not
satisfy any clause of this contract I may be administratively withdrawn from Guilford.
2.
I will keep my contact information up-to-date in my Banner Web account by promptly informing the Registrar’s office of any and all
changes.
3.
I will establish and check my Guilford email account by the first day of classes.
4.
I will meet with my SSM at least ½ hour each week (or more as determined by my SSM) to discuss my progress. I understand that I must
give at least 24 hours notice if I need to reschedule an appointment and that missing and/or being late to a meeting is a violation of this
contract.
5.
To be as successful as possible, I understand that the college has strongly encouraged me to discuss and disclose my disabilities, if any,
with Disability Resources (located on the second floor of the library) and to utilize all provided accommodations.
6.
I will obtain all the necessary books and supplies for my courses as instructed by my teachers and their course syllabi.
7.
I will attend all of my classes. I understand that if I am too sick to attend class, I must present appropriate medical documentation to my
instructor(s) and my SSM.
8.
I will earn a grade of “B-“ or better in the one-credit GST 120 Learning Strategies class in the first half of the first semester.
9.
If eligible to enroll in the one-credit FYE 102 course, I will enroll in this course and earn a grade of “C“or better .
10. I understand that as with all beginning students, if I have a cumulative G.P.A. greater than 1.0 after one semester, I will be allowed to
continue for at least another semester at Guilford College. If not, I realize that I will be academically dismissed.
11. I understand that, as with all students, if my cumulative G.P.A. is below 2.0, I will be on academic probation. I further understand that
failure to obtain a term G.P.A. of 2.0 while on academic probation will result in academic suspension or dismissal.
12. I understand that if I am placed on academic probation I will continue to meet with my SSM at least ½ hour each week (or more if
determined by my SSM) until I get off of academic probation.
13. I will adhere to all of the college’s academic honor code and judicial policies.
14. I understand that as a part of SSP I may be asked to meet with college personnel associated with this program. Attendance standards for
these meetings are the same as those for classes.
15. I grant permission to Guilford College, its officers, agents, and employees, to release any and all academic and other information
maintained or contained in the files concerning my academic performance, college living arrangements and personal conduct, which
information may be requested by my parents or other Guilford College offices.

Other conditions may be added after initial conference with your Student Success Mentor. An amended copy will be
forwarded to the student and her/his parents.
