Certification Assessments (MCAs),
along with a total count. The second
and third tabs provide a list of
participants certified or who have had
a MCA completed (respectively)
during specified dates.
Tab 1 – Count: Count of initial and
follow-up contacts.
Tab 2 – Cert/Recert Initial & FU: List
of participants who were certified.
Tab 3 – MCA Initial & FU: List of
participants who had MCAs
completed.
*Contacts are based on the MA/MN Care active-CTC Referral
Given Local Use Question #1. Initial CTC Contacts are based on
selection of YES, INITIAL (PASSPORT); Follow-up CTC Contacts
are based on selection of YES, FOLLOW-UP (BROCHURE).
*The MCA Initial & FU report output is based on completion of
the MCA Guided Script. Any MCAs completed via the Participant
Folder will not be included in this report.
*The Agency ID is based on where the participant was certified in
the Cert/Recert Initial & FU report output and where the
participant’s MCA was completed in the MCA Initial & FU report
output.
*The Contact Date is the date the participant of the participant’s
most recent certification in the Cert/Recert Initial & FU report
output and the date the participant’s MCA was completed in the MCA Initial & FU report output.

Notes
A horizontal table was created for Tab 1 – Counts.
The link between the MEMBER and CERTCONTACT tables was changed from Certificationid to Statewicid; the
Certificationid updates in the MEMBER table when pseudo-certs are created.
The CGS Initial, MCA Initial, CGS Follow-up, and MCA Follow-up queries were used for the counts in Tab 1. The
Both and MCA Both queries were used for the detail reports in Tab 2 and Tab 3.
