Non-Walter Johnson High School Guest Form
Middle School Students are not permitted
This form must be complete and returned
before tickets can be picked up for non-WJHS guest
Instructions:
1. All guests must be 20 years of age or younger but not in middle school
2. Page 2 of this form must be completed by the WJ student and guest
3. A Principal or Asst Principal at the guest’s school must sign the Guest School Administrator
Approval section of this form
4. The completed form must be returned to Ms. Regina Rodriguez, WJ Senior Class
Administrator, no later than Wednesday, April 27, 2016
5. All guests must bring a valid picture ID in order to be admitted to the prom:
• Student ID required for guests from another high school
• Student ID or government issued ID (example: Driver’s License) for guests who are in
college or working
6. Students will be permitted to pick up tickets, as long as they have no financial obligations, once
Ms. Rodriguez receives this approved form
7. Tickets are non-transferable. All sales are final – no refunds. Tickets will not be sold at the
door.
